Steps To Enrollment

• Potential FOCUS participants will fill out the FOCUS Interest Form and return it to the HISRA office.
• Potential FOCUS participants will come in and meet with Program Staff to evaluate eligibility for the program.
• A joint family discussion will take place between program staff, the potential participant and his/her family members to determine if the program meets the needs of the participant and if the participant meets minimum requirements of the program.
• Attend the FOCUS program for a trial period to determine eligibility and to ensure the potential participant enjoys the program (length of trial period can be arranged with the Management Team).
• Follow up with the Program Specialist at the end of the trial period to assess how it went and if HISRA staff, the potential participant, and family agree on enrollment. Then decide how many days per week to enroll. If the desired days are full, the participant will be put on a waitlist.
• Start on the date set; billing arrangements have been made.

Annual Information Forms Disclaimer

All HISRA participants must have a current 2025 Annual Information Form, and/or any supplementary forms on file in order to participate in programs at HISRA. Any participants that do not have a current Annual information Form on file will not be permitted to HISRA programs. Failure to complete forms in a timely manner will result in a suspension from programming without refunds or credits. All HISRA forms can be found at www.hisra.org/forms.
