COVID-19 Program Screening Info
Prior to joining an indoor or outdoor program with HISRA or entering the HISRA building, the program participant or responsible party must conduct a wellness assessment that includes the following:
Do you have any of the following?
□ Fever or chills • (100.4° F [38.0° C] or greater using an oral thermometer), have signs of a fever, and any other symptoms for at least 24 hours, without the use of fever-reducing or other symptom-altering medicines (e.g. cough suppressants).
□ Cough
□ Shortness of breath or difficulty breathing
□ Fatigue
□ Muscle or body aches
□ Headache (unusual to the person)
□ New loss of taste or smell
□ Sore throat
□ Congestion or runny nose
□ Nausea or vomiting
□ Diarrhea
□Are you ill, or caring for someone who is ill? •Participants who are well but who have a sick family member at home with COVID-19 not attend programs.
- If a participant is confirmed to have COVID-19, they should inform HISRA of their illness if they have attended a program recent to their diagnosis
□ Have contact with someone diagnosed with COVID-19?
Participant should not attend program if YES is answered to any of the above symptoms or questions.
